During a fictional payment-change discussion, the team could describe how it would verify a request. It could not locate every supporting procedure. Here is a useful way to record that difference.

- Verify changed vendor payment instructions while the usual approver is unavailable.
- Observation
- The fictional note-taker located an approved vendor-contact record. No backup approval role was identified during discussion.
- Evidence located
- The approved record’s location was identified in the exercise. No real contact was called and no payment system was opened.
- Not tested
- Email authentication, MFA, account permissions, whether the contact is still current, recovery, actual approval or transfer controls.
- Unknown
- Which authorized role handles this request when the usual approver is unavailable?
- Interim decision
- Keep the sensitive change paused and follow the existing escalation procedure. Urgency does not establish authority.
- Evidence requested
- A current, owner-approved backup authorization procedure and confirmation of where staff can find it.
- Follow-up owner
- Fictional agency owner role, working with the existing provider where needed.
- Review date
- October 29, 2026 — illustrative date only, not an appointment or a promised completion date.
- Status
- Open in this fictional record. No correction or control effectiveness is claimed.
Keep the conclusion narrow.
“No backup role was identified during discussion” is an observation. It does not establish that no backup exists, that an agency was breached or that an exercise made it secure.
